Become a Patient

We'd love to help you, your child, or loved one reach their functional goals. In order to become a patient, the individual will need to have a current occupational therapy evaluation (completed within the last 365 days). Evaluations can be completed by any licensed occupational therapist. We are able and happy to provide evaluations at the barn with a doctors referral.

 

Please send referrals or current evaluations via email to kelsey@healingstepstherapy.org along with new patient paperwork found under the "become a patient tab" in the menu. Be sure to check the Precautions document prior to sending us your paperwork to be sure equine activities are a safe match for the patient's conditions. Once we receive the evaluation, we will be in touch to discuss if Healing Steps is appropriate to assist in reaching your goals.
